Resolution Health - Chronic illness benefit cover 2012

All Plans provide cover for the 27 prescribed Chronic Disease List (CDL) conditions at 100% Scheme Rate. This cover needs to be applied for via the Pharmaceutical Department (Hospital plan) or SwiftAuth Online (Progressive plan). On the Hospital plan Chronic Medication must be obtained from the State and on the Progressive plan it is not limited to a Designated Service Provider (DSP) but is subject to a formulary.
