There is no annual limit for hospitalisations, and the benefits include certain specialised procedures & treatments. There is two custom hospital plan options available, State For Chronic* and Any For Chronic**.

State For Chronic members must use state hospitals to get full cover for treatment of 26 Chronic Disease conditions. As an incentive Momentum has reduced the plans monthly contributions for any inconvenience.

Any For Chronic members can use any Associated Network hospital for chronic disease treatment. There is an increase in the monthly contribution cost for this added convenience.
